Despite an earlier report from very well-respected fracture surgeons showing excellent results from interlocking nailing, this option is still usually not considered.[1] Although patients with distal tibial metaphyseal fractures may be cared for nonoperatively, these fractures can be very unstable and difficult to control in a cast. Additionally, many distal tibial fractures are associated with lateral malleolar fractures, and the need for fixation of the distal fibula remains controversial.
